DUI Pre-Registration Forms

The Florida Safety Council’s DUI Counterattack program offers online pre-registration as a way to speed up the process when registering in person. In order to pre-register you must complete the Pre-Registration Form and review the Program Rules. You may print these forms from this site.

After reading the Program Rules, Student Conduct Form, Confidential Information Form and completing the Pre-Registration Form, you may bring the completed forms with you when you come to register. You must register in person.

The items listed below must be included with the signed Program Rules & the Pre-Registration Form when you come in to register.

You will not be scheduled for class or evaluation until the DUI Program has received the following items:

  1. Driver License Number
  2. Arrest Affidavit (Arrest Report or Traffic Crash Report) - must be a legible copy
  3. Citation – must be a legible copy
  4. Results of Breath or Blood test, Urinalysis results, Refusal Slip
  5. Court Disposition or Court Order – (if have already appeared in Court or been convicted)
  6. Picture ID-- such as a current Florida Identification Card, US Passport. (NOT EXPIRED)
  7. Complete, officialLifetime Florida Driving Record (call for information on official sources)

Failure to provide the above information will result in a delay of enrollment until provided.

The complete, official Lifetime Florida Driving Record must be provided by the client at the time of the evaluation appointment.  If the Lifetime Driving Record is supplied by the client, the Driving Record must be an official DHSMV-based printout, less than 30 days old, and reflect a lifetime record.  If another state issued the Driver License, the lifetime record from that state must also need to be presented.

  1. **Clients that have a Florida Driver's License Number may pay an additional fee of $14.00 (cash, credit card or money order) at the time of the evaluation appointment to have the Lifetime Driving Record processed by Florida Safety Council.

  2. **Clients may also contact the DHSMV, Clerk of Court or an Internet resource to obtain a current copy of the Lifetime Driving Record.

Upon completion of the above items along with the signed Program Rules, Student Conduct Form and Confidential Information Form you will be ready to come in person to any of our locations to complete the registration process. At the time of registration you will be scheduled for the appropriate DUI class and evaluation.
